The Service
Vulnerable people discharged home on their own, who do not have immediately apparent nursing or social care needs, are contacted by a member of CVS staff within 48 hours of discharge to check how they are coping. This may be by telephone or a home visit. If problems have arisen, they will then be referred on to the appropriate service.
Contacts
Referrals are normally made shortly prior to discharge by ward staff (with the consent of the patient) to the co-ordinator at the hospital, who then passes details on to our Home from Hospital Co-ordinator, Jill Lawson.
