This is an award winning scheme based on the GP practice in Kirkby
Malzeard and Masham.
The Service
Patients are referred to the scheme if they need information or practical help to enable them to stay in their own homes, or if they need support to maintain good health or prevent deterioration. The scheme Co-ordinator will visit to ascertain whether patients are getting all the help they are entitled to from statutory services, and can make referrals for assessments from Social Services, Occupational Therapists, and other agencies. Where there are quality of life issues not catered for by statutory services, the Co-ordinator will arrange voluntary help. There is also a Carers’ Group, offering support to people caring for sick or disabled dependants.
Volunteers
Volunteers drive patients to appointments, visit them at home, do practical tasks such as shopping, taking out for walks or writing Christmas cards, and sit with patients who cannot be left alone to allow carers a break.
